This form is a mail-in application for Medicaid health care coverage for individuals with disabilities who are employed. There are two groups of coverage: Worker with a disability, and Worker with a Medically Improved Disability. Individuals determined eligible for the program must pay a monthly premium in order to receive the health care coverage.
